Destiel Media Release Form

This Media Release Form ("Form") is made between Destiel (the "Organization") and the undersigned volunteer (the "Volunteer"). By signing this Form, the Volunteer grants permission to the Organization to use their name, likeness, and any photos or videos taken during their service for promotional and marketing purposes.

1. Purpose

To obtain the Volunteer's consent for the use of their name, likeness, and any photos or videos taken during their service in various media formats.

2. Grant of Rights

The Volunteer grants the Organization the following rights:

The right to use, reproduce, distribute, and display the Volunteer's name, likeness, and any photos or videos taken during their service.
The right to edit, modify, or adapt the photos or videos as necessary for the Organization's promotional and marketing purposes.

3. Scope of Use

The Volunteer agrees that their name, likeness, and any photos or videos taken during their service may be used in the following media formats:

Print materials, such as brochures, flyers, and posters.
Digital media, including the Organization's website, social media accounts, and email newsletters.
Multimedia presentations and promotional videos.

4. Duration of Use

The rights granted to the Organization are perpetual and may be used for an indefinite period unless the Volunteer revokes this consent in writing.

5. No Compensation

The Volunteer understands and agrees that they will not receive any monetary compensation, royalties, or other benefits for the use of their name, likeness, and any photos or videos taken during their service.

6. Release of Liability

The Volunteer releases the Organization from any and all claims, demands, or causes of action that may arise from the use of their name, likeness, and any photos or videos taken during their service.

7. Revocation of Consent

The Volunteer may revoke this consent at any time by providing written notice to the Organization. Upon receipt of the notice, the Organization will cease using the Volunteer's name, likeness, and any photos or videos taken during their service in future materials.

8. Acknowledgment and Acceptance

By signing below, the Volunteer acknowledges that they have read, understood, and agree to the terms and conditions outlined in this Media Release Form.
