Destiel Volunteer Verification Consent Form

This Volunteer Verification Consent Form ("Form") outlines the terms and conditions under which the undersigned volunteer (the "Volunteer") agrees to have their information included in Destiel's Volunteer Verification System (VVS).

1. Purpose

The Volunteer Verification System is designed to help us verify the contributions and status of our volunteers in a simple and secure manner. This ensures transparency and maintains the integrity of our organization.

2. Information Included

The system will display the following information about the Volunteer:

  • Name
  • Email address (if necessary)
  • Start date
  • Departure date (if applicable)
  • Feedback status (positive, neutral, or negative note upon departure)
  • Current status (active or inactive)
  • Total hours contributed
  • Any other details as deemed necessary

3. Consent

By signing this form, the Volunteer consents to having the above information included in the VVS and available for search by community members and any interested parties. Consent to be included in the VVS is a mandatory condition for volunteering with Destiel. Volunteers who do not consent to this cannot be part of the organization.

Irrevocability of Consent

The consent granted by the Volunteer is irrevocable. This is necessary to:

  • Ensure transparency and accountability: Accurate and publicly verifiable records help maintain the trust and integrity of the organization.
  • Validate volunteer contributions: The VVS helps verify the efforts and status of volunteers, preventing false claims about involvement.
  • Maintain accurate records: Having a complete and permanent record of volunteer activities supports the effective operation of the organization.

4. Privacy and Security

  • Controlled Access: The system does not display a list of all volunteers. Only those who are searched for specifically will have their information displayed.
  • Inquirer Details: Individuals inquiring about volunteers must enter their details (name, email address) for traceability reasons. Their IP address will also be logged.
  • Limited Information: The search results will only show specific information necessary for verification, such as the volunteer's current status, start date, departure date (if applicable), total hours contributed, and feedback status. This protects the volunteer’s privacy.

5. Timesheet System

Volunteers are required to use the timesheet system to clock in and clock out daily and to note in detail the tasks performed each day. This information helps us track contributions and ensure accurate verification.

6. Acknowledgment and Acceptance

By signing below, the Volunteer acknowledges that they have read, understood, and agree to the terms and conditions outlined in this Volunteer Verification Consent Form. They understand that inclusion in the VVS is a condition of volunteering with Destiel.
